Thyroid function in cerebral vascular accidents
Yoshikawa et al., Osaka, Japan, investigated TSH and prolactin response to TRH in 15 patients with cerebral infarction (CI) and 13 with cerebral haemorrhage (CH). They found that in patients with CH delta TSH was not significantly different from normal but decreased significantly in CI patients. Delta T3 was normal in both patients groups as was delta prolactin. The authors conclude that in CI, but not in CH patients a selective ischemia of TSH- secreting pituitary cells is present, because despite an impaired response of TSH after TRH, prolactin response was normal. (Comment: These results are interesting but strange is that, despite an impaired delta TSH in CI patients delta T3 was normal. How is this going to be explained?)
